About Vitamin E (alpha-tocopherol)

The fat-soluble antioxidant vitamin, supplied as natural d-alpha-tocopherol or synthetic dl-alpha-tocopherol (a mixture of eight stereoisomers, roughly half as potent per milligram). Most capsules contain alpha-tocopherol alone, which is only one of the eight compounds that make up dietary vitamin E.

Vitamin E was the great antioxidant hope of the 1990s and it did not survive contact with randomised trials. Deficiency is genuinely rare outside fat-malabsorption disorders and specific genetic conditions, and supplementation on top of an adequate diet has failed to deliver on cardiovascular disease, cancer or cognition. Worse, the harms accumulated: a dose-response meta-analysis linked doses of 400 IU/day and above with higher all-cause mortality, a meta-analysis of stroke subtypes found more haemorrhagic strokes, and the SELECT trial found 17% more prostate cancer in men taking 400 IU/day. Its remaining defensible use is correcting deficiency in malabsorption, which is a clinical situation, not a shelf product.

Regulation: The IOM set a Tolerable Upper Intake Level of 1,000 mg/day of any form of supplemental alpha-tocopherol, based on bleeding risk. The US Preventive Services Task Force recommends against vitamin E supplementation for cardiovascular disease or cancer risk reduction. Labels convert inconsistently between IU and mg — 1 mg of natural d-alpha-tocopherol is 1.49 IU, synthetic is 2.22 IU per mg — which is a common source of accidental overdosing.

What is in a retail bottle is frequently not what was studied, and the amount that suits one person is not general advice.

    15 mg/day of alpha-tocopherol. Typical Western diets fall somewhat short of this on paper, without producing any measurable deficiency disease — a useful reminder that an RDA is a population-planning figure, not a clinical threshold.

    Set on haemorrhagic risk. Note that most trials showing harm used 400 IU/day — roughly 180–270 mg depending on form — which is well below this ceiling.

    Absorption requires bile and dietary fat, and is poor when taken fasted. Established pharmacology from absorption studies rather than a comparative outcome trial.

The evidence in depth

What each association actually rests on, and the papers behind it. A rating describes the state of the research, not how promising the supplement sounds. How we rate evidence.

In non-alcoholic steatohepatitis without diabetes, 800 IU/day improved liver histology in the PIVENS trial. It is the most positive vitamin E result in the modern literature, it applies to a specific diagnosis under specialist care, and it uses a dose with documented long-term risks.

Supplementation reliably lowers some laboratory markers of lipid oxidation. Whether moving those markers corresponds to any health outcome is exactly what the large trials tested, and the answer was no.

What may be going on: Alpha-tocopherol terminates lipid peroxidation chain reactions in cell membranes, which is straightforward chemistry and the reason the hypothesis was worth testing.

Not enough evidence

Multiple large randomised trials and their meta-analyses found no reduction in cardiovascular events from vitamin E supplementation, and the HOPE-TOO extension found more heart failure in the vitamin E arm. This is a well-replicated null with a harm signal attached.

There is not enough consistent evidence that vitamin E slows cognitive decline. One trial in mild-to-moderate Alzheimer disease reported slower functional decline at 2,000 IU/day, but it has not been reliably replicated and that dose sits in the range associated with excess mortality.

Things to know

Interactions, contraindications and dose limits, most serious first. Concentrated extracts interact with medication in ways the whole plant does not, so this section carries more weight here than it does on a food page.

  • Too much

    Increased haemorrhagic stroke and increased all-cause mortality

    A meta-analysis of stroke subtypes found a 22% relative increase in haemorrhagic stroke with vitamin E supplementation, alongside a smaller reduction in ischaemic stroke. Separately, a dose-response meta-analysis found higher all-cause mortality at doses of 400 IU/day and above. These are pooled findings across tens of thousands of participants, not isolated results.

  • Too much

    Increased prostate cancer in SELECT

    The SELECT trial randomised more than 35,000 men to 400 IU/day of vitamin E and found a 17% increase in prostate cancer incidence over seven years. The trial was designed to show the opposite. Men should know this before taking vitamin E for any reason.

  • Interaction

    Bleeding risk with anticoagulants and antiplatelets

    Interacts withWarfarin, direct oral anticoagulants, aspirin, clopidogrel

    Vitamin E antagonises vitamin K–dependent clotting factors and inhibits platelet aggregation, and the upper limit is set on that basis. Combined with warfarin, direct oral anticoagulants, aspirin or clopidogrel it adds to bleeding risk, and high doses are generally stopped before surgery.

  • Interaction

    May reduce the effect of some chemotherapy and statin-niacin combinations

    Because several cancer therapies work partly through oxidative mechanisms, high-dose antioxidants during treatment are generally discouraged pending better data. An antioxidant cocktail including vitamin E also blunted the HDL response to simvastatin plus niacin in one trial.

  • Worth knowing

    Alpha-tocopherol alone displaces the other tocopherols

    High-dose alpha-tocopherol lowers blood gamma-tocopherol, the form that predominates in most diets. Whether that matters is unresolved, but it means an "isolated vitamin E" capsule shifts the balance of dietary vitamin E rather than simply adding to it.

The whole-food form, where one exists. The evidence for a concentrated extract rarely transfers to the food, or the other way round — these are cross-links, not equivalents.
