Large well-conducted trials found saw palmetto no better than placebo for urinary symptom scores, and the Cochrane review reached the same conclusion. Some European trials of a specific hexanic extract report benefit, so the question is not entirely closed, but the weight of the best evidence is negative.
What may be going on: Proposed to inhibit 5-alpha-reductase and bind androgen receptors, though prostate tissue studies have not consistently shown this at supplement doses.
Limited evidence. Early human work, or mostly lab and animal research.
