The single naturally occurring enantiomer of lipoic acid, sold either free or as the more stable sodium salt. Ordinary supplements contain a fifty-fifty mixture of this and the synthetic S form.
The argument for R-lipoic acid is straightforward: only the R form is made by the body and used by the mitochondrial enzymes, so the S half of a racemic capsule is at best inert. Pharmacokinetic studies do show higher plasma peaks from the R form and from the sodium salt in particular. What does not exist is any clinical trial comparing R-lipoic acid with the racemic mixture on an actual outcome — the entire neuropathy and metabolic literature was built on the racemic material. Free R-lipoic acid is also chemically unstable, polymerising with heat and moisture, which is why stabilised salts dominate the better products.
Regulation: Sold as a dietary supplement in the US without pre-market efficacy review, at a substantial price premium over racemic material that no outcome trial supports.
