About Propolis

A resinous material bees collect from tree buds and sap and use to seal and sterilise the hive. Its chemistry depends entirely on the local flora, with poplar-type, Brazilian green and red propolis being chemically distinct.

Propolis has a better mechanistic case than most bee products — it is genuinely antimicrobial in the hive and in laboratory assays, and it contains caffeic acid phenethyl ester and a range of flavonoids. Human evidence exists but is patchy: the most consistent findings are for topical or mouthwash applications in oral mucositis and dental plaque, with oral supplement trials for immune and metabolic outcomes small and inconsistent. The category problem is chemical: "propolis" from Brazil, China and Europe are different substances with different marker compounds, so the dose on one bottle means something different from the dose on another. Contact allergy is the commonest adverse effect and is well documented, particularly among beekeepers.

Regulation: Sold as a dietary supplement in the US with no pre-market efficacy review. Some topical and dental propolis products are regulated as cosmetics or medical devices depending on jurisdiction and claim.

Meta-analyses of small trials in type 2 diabetes report modest reductions in fasting glucose and HbA1c. The trials are small, heterogeneous in propolis type, and mostly from a few countries.

Small trials report changes in immune markers and some reduction in upper respiratory symptoms, particularly with throat sprays. The studies are small, the propolis types differ between them, and the results are not consistent.

What may be going on: Propolis flavonoids and caffeic acid derivatives have antimicrobial and anti-inflammatory activity in laboratory assays, including against oral bacteria.

  • Allergy

    Contact allergy and anaphylaxis

    Propolis is a well-recognised contact allergen and one of the more frequent causes of positive patch tests among natural products, especially in beekeepers. Ingestion has caused oral mucosal reactions and, less commonly, anaphylaxis. Anyone with bee product allergy or a poplar and balsam of Peru sensitivity should avoid it.

  • Worth knowing

    Propolis is not one substance

    Poplar-type European propolis, Brazilian green propolis and Cuban red propolis have different marker compounds and different biological activity. A milligram figure carries no meaning without knowing which type it is, and trials cannot be pooled across them as if they tested the same thing.

  • Interaction

    Anticoagulant and enzyme interactions

    Propolis inhibits platelet aggregation in laboratory work and inhibits several cytochrome P450 enzymes, so interactions with anticoagulants and with medicines cleared by CYP3A4 are plausible.

  • Avoid

    Asthma and pregnancy

    People with asthma are advised to be cautious with bee products generally given the reaction risk, and there is no safety data for propolis in pregnancy or breastfeeding.
