About Phosphatidylserine

A phospholipid concentrated in the inner leaflet of neuronal cell membranes. Supplements were originally extracted from bovine brain cortex and are now almost entirely made from soy or sunflower lecithin.

Phosphatidylserine has an unusual regulatory history that tells the story better than any summary: the FDA permitted a qualified health claim for cognitive decline while stating in the same document that there is little scientific evidence supporting it. The trials that generated the original enthusiasm in the late 1980s used bovine-cortex-derived phosphatidylserine, which was withdrawn over transmissible spongiform encephalopathy concerns. The plant-derived material that replaced it has a different fatty-acid composition and has performed less well, so the modern product is not the one the classic studies tested.

Regulation: The FDA permits two qualified health claims for phosphatidylserine and cognitive dysfunction or dementia in the elderly, both of which must be accompanied by the statement that the agency concludes there is little scientific evidence supporting the claim. Bovine-derived phosphatidylserine was removed from the market over BSE risk.

    The classic dose, from trials using bovine cortex material that is no longer sold. Soy-derived phosphatidylserine at the same dose has produced smaller and less consistent results.

    Small trials in athletes reported blunted cortisol responses to intense exercise. Sample sizes are in the tens and several were supported by the ingredient manufacturer.

Trials in older adults with memory complaints report modest improvements on some memory measures, most clearly with the bovine-derived material that is no longer available. Soy-derived phosphatidylserine has produced weaker results, and the FDA describes the supporting evidence as limited when permitting the claim.

What may be going on: Phosphatidylserine is a structural phospholipid of neuronal membranes and a cofactor for several membrane-bound signalling enzymes. Whether oral supplementation changes brain phosphatidylserine content in humans is not established.

    Phosphatidylserine has been reported to interact with cholinergic medication used in dementia, and combining it with acetylcholinesterase inhibitors is usually flagged for prescriber review.

    Most products are derived from soy lecithin. Highly processed lecithin contains little residual protein, but sunflower-derived alternatives exist for anyone with a soy allergy.

    The most positive trials used bovine cortex phosphatidylserine, withdrawn over prion transmission concerns. Transferring those results to a soy-derived product is an assumption, not a finding.
