Manuka honey is unusual in having a genuine, measurable and chemically specific antibacterial property: dihydroxyacetone in manuka nectar converts to methylglyoxal during maturation, giving an antibacterial activity that survives dilution and is graded as UMF or MGO. Where this has clinical support is topical — sterilised medical-grade honey dressings are regulated wound-care products with a real evidence base. Swallowed, the picture is very different. Trials of oral manuka honey for coughs, sore throats, digestive symptoms and Helicobacter are small and largely unconvincing, and any honey soothes a cough about as well. The commercial problem is fraud: far more honey is sold as manuka worldwide each year than New Zealand produces.

Honey reduces cough frequency and severity in children compared with no treatment in several small trials, an effect attributed to its viscosity and sweetness rather than to manuka chemistry. Manuka-specific respiratory evidence is thin.

Avoid
Never give honey to an infant under twelve months
Honey can contain Clostridium botulinum spores, which germinate in an infant gut that lacks a developed microbial community and cause infant botulism — a serious and occasionally fatal illness. This applies to all honey including manuka and to any product containing it, and there is no grade or processing that makes it safe for a baby.

Widespread mislabelling
Several times more honey is sold internationally as manuka than New Zealand produces. UMF or MGO grading from a certified supplier is the only meaningful assurance, and unrated "manuka" honey may be ordinary honey at several times the price.
Too much
It is sugar
Manuka honey is roughly 80% sugar. Taking spoonfuls daily as a health measure adds meaningful free sugars, which matters particularly for anyone with diabetes or managing weight, and the honey's effect on blood glucose is not softened by its grading.
Allergy
Bee product and pollen allergy
Honey contains pollen and bee-derived proteins, and allergic reactions including anaphylaxis have been reported in sensitised people.

Do not use kitchen honey on a wound
The wound-care evidence uses gamma-irradiated medical-grade honey, sterilised specifically to remove bacterial spores. Applying a jar of food honey to an open wound is not the same intervention and introduces contamination risk.
