About Magnesium malate

Magnesium bound to malic acid, an intermediate in the citric acid cycle, supplying roughly 15% elemental magnesium. It is marketed for fatigue and muscle pain on the strength of malate's role in energy metabolism.

Magnesium malate is a reasonable, well-tolerated magnesium salt sold on a claim it has not earned. The fibromyalgia and fatigue marketing traces to a small trial in the 1990s using malic acid with magnesium, which reported symptom benefit only at doses far above those in retail products and has not been replicated. The malate component is a normal metabolic intermediate that the body produces continuously in every cell, which makes the idea that supplying a few hundred milligrams meaningfully changes energy metabolism difficult to sustain. As a magnesium source it is fine; as a fatigue intervention it is unsupported.

    Labels often state the total salt weight, which is roughly seven times the elemental magnesium figure. The malate content of retail products is a fraction of what the fibromyalgia study used.

    One small trial from the 1990s. The blinded phase was negative at lower doses; benefit appeared in an open-label extension at higher doses, which is a much weaker design. It has not been replicated.

As a well-absorbed magnesium source it should share magnesium's modest blood pressure effect, which meta-analyses put at around 2 mmHg systolic. No trial has tested the malate form specifically.

    Kidney impairment

    Magnesium accumulates when kidney function is reduced, and supplements have caused serious hypermagnesaemia in that setting.

  • Interaction

    Reduces absorption of antibiotics, bisphosphonates and levothyroxine

    Separation by two to four hours is standard practice with tetracyclines, fluoroquinolones, bisphosphonates and thyroid hormone.

    Laxative effect and the upper limit

    All magnesium salts cause loose stools above a personal threshold, which is the basis of the 350 mg/day supplemental limit.

  • Worth knowing

    Fatigue deserves investigation

    Persistent fatigue is more often explained by anaemia, thyroid disease, sleep apnoea, depression or medication than by magnesium status. Using a supplement in place of assessment delays finding the cause.
