About L-arginine

A conditionally essential amino acid and the direct substrate for nitric oxide synthase. Sold widely as a "nitric oxide booster" for blood flow and pump.

Arginine is a case where a correct mechanism is defeated by pharmacokinetics and where a trial found real harm. Oral arginine is extensively degraded by arginase in the intestine and liver, so a large oral dose raises plasma arginine much less efficiently than citrulline does — which is why citrulline has largely replaced it in serious formulations. More importantly, the VINTAGE MI trial added arginine to standard therapy after myocardial infarction in older patients and was stopped early: six participants in the arginine group died and none in the placebo group. That result belongs on any page discussing arginine, and it is rarely mentioned where it is sold.

Regulation: Sold as a dietary supplement with no pre-market efficacy review. Also used clinically as an intravenous agent in growth hormone stimulation testing, which is a diagnostic procedure and not related to the supplement use.

What is in a retail bottle is frequently not what was studied, and the amount that suits one person is not general advice.

    Meta-analyses report small blood pressure reductions across a very wide dose range. Gastrointestinal upset limits tolerance above roughly 10 g in a single dose.

    Results are inconsistent, and the first-pass degradation of oral arginine is the usual explanation. Citrulline raises plasma arginine more effectively at lower doses.

    Often taken away from protein-containing meals because other amino acids compete for the same transporter. This is transport pharmacology rather than a tested outcome comparison.

Meta-analyses of oral arginine report modest reductions in systolic and diastolic blood pressure. Trials are heterogeneous in dose and duration, and the effect is smaller than the nitric oxide mechanism would predict because most oral arginine never reaches the circulation intact.

What may be going on: Arginine is the substrate for nitric oxide synthase, but intestinal and hepatic arginase degrade a large fraction of an oral dose before it enters the systemic circulation.

Trials of oral arginine for strength, endurance or blood flow during exercise are inconsistent and largely negative, which is consistent with its poor oral bioavailability. There is not enough evidence to support a performance claim.

  • Avoid

    Increased mortality after myocardial infarction in a randomised trial

    The VINTAGE MI trial added L-arginine to standard post-infarction care in patients aged 60 and over and was terminated early for safety: six deaths occurred in the arginine group and none on placebo, with no improvement in vascular stiffness or ejection fraction. Arginine should not be taken after a recent heart attack, and this finding is almost never surfaced where the supplement is sold.

    Additive hypotension with nitrates and PDE5 inhibitors

    Interacts withNitrates, sildenafil and other PDE5 inhibitors, antihypertensives

    Arginine acts on the nitric oxide pathway, the same pathway as nitrate drugs and sildenafil-type medicines. Combining them can cause a substantial drop in blood pressure.

  • Worth knowing

    Herpes reactivation

    Herpes simplex virus replication is arginine-dependent in laboratory work, and arginine supplementation is commonly reported to precipitate outbreaks in susceptible people. The evidence is largely anecdotal but the mechanism is well described.

  • Too much

    Gastrointestinal upset at effective doses

    Nausea, cramping and diarrhoea are common above about 10 g in a single dose, and are the practical ceiling on oral arginine use.
