About High-DHA omega-3 concentrate

A marine oil concentrated so that DHA predominates, sold for brain, eye and pregnancy uses. Sourced from fish oil fractionation or from algae.

DHA is the structural omega-3 — it is concentrated in neural and retinal membranes, and it is the fatty acid that accumulates in the fetal brain in the third trimester. That structural role is real and is the strongest argument for DHA in pregnancy and infancy. It has not translated into much for adult cognition: trials of DHA in older adults report small or absent effects on memory, and the largest eye trial of its kind, AREDS2, found no benefit of added EPA and DHA for progression of age-related macular degeneration. DHA-rich oils also raise LDL cholesterol slightly more than EPA-rich ones.

Regulation: Sold as a dietary supplement in the US with no pre-market efficacy or safety review. EFSA permits a claim that DHA contributes to maintenance of normal brain function and normal vision at 250 mg/day, which is a maintenance-of-normal-function claim and not a claim of improvement.

DHA lowers triglycerides at least as effectively as EPA and raises HDL slightly, but it also raises LDL cholesterol modestly. Anyone choosing between concentrates on lipid grounds should know the trade-off runs in both directions.

DHA is a major structural lipid of photoreceptor outer segments, and observational data link higher fish intake to lower macular degeneration risk. The large AREDS2 randomised trial added EPA and DHA to the antioxidant formulation and found no reduction in progression, which is the strongest single piece of evidence available and it is negative.

Some trials in older adults with subjective memory complaints report modest gains on learning and memory tasks at around 900 mg DHA/day. Trials in diagnosed Alzheimer disease have been null, and meta-analyses describe the overall evidence as inconsistent.

What may be going on: DHA is the most abundant polyunsaturated fatty acid in neuronal membranes and influences membrane fluidity and synaptic signalling. A structural role does not by itself imply that adding more changes function.

    The same platelet effects apply to DHA-rich concentrates as to any long-chain omega-3, and concentrates make multi-gram doses easy to reach.

    Atrial fibrillation signal at high dose

    The dose-dependent increase in atrial fibrillation seen in REDUCE-IT, STRENGTH and OMEMI applies to total omega-3 intake, not to a particular ratio. Doses above about 1 g/day are where the signal appears.

    LDL cholesterol rises modestly

    DHA-predominant preparations raise LDL cholesterol slightly more than EPA-predominant ones. In someone taking omega-3 specifically for cardiovascular reasons this is worth knowing and is rarely disclosed at the point of sale.

    Tuna-derived DHA concentrates come from a larger, longer-lived fish, which raises the importance of contaminant testing, and concentrates oxidise more readily than dilute oils.
