Preparations of protease, amylase, lipase and specialty enzymes such as lactase or alpha-galactosidase, sourced mostly from fungal fermentation. Sold to be taken with meals to aid digestion.

This category contains one very well-evidenced intervention and a lot of speculation sold alongside it. Pancreatic enzyme replacement is a prescription therapy that unequivocally works for exocrine pancreatic insufficiency in cystic fibrosis, chronic pancreatitis and after pancreatic surgery — a genuine deficiency correction. Lactase for lactose intolerance and alpha-galactosidase for the oligosaccharides in beans are also enzymatically straightforward and supported by trials. General-purpose enzyme blends taken for bloating by people with normal pancreatic function are a different proposition: there is little evidence they help, and the healthy pancreas already secretes these enzymes in large excess.

Regulation: Pancrelipase is a prescription drug in the US and EU for exocrine pancreatic insufficiency, with mandated potency standards. Over-the-counter enzyme blends are dietary supplements with no pre-market efficacy review, and their activity units are not comparable to prescription lipase units.

Lactase reduces symptoms after lactose ingestion in lactose-intolerant people, and alpha-galactosidase reduces flatulence after bean meals — both supported by trials and both narrow, mechanistically clear interventions. Broad-spectrum blends taken for general bloating have very little supporting evidence.

What may be going on: Each enzyme hydrolyses a specific substrate in the intestinal lumen before it reaches colonic bacteria, removing the fermentable material that produces gas.

There is not enough evidence that enzyme blends change bowel habit in people without pancreatic insufficiency. In genuine exocrine pancreatic insufficiency, prescription enzyme replacement resolves steatorrhoea, which is a different clinical situation entirely.

    Not a substitute for prescription enzyme replacement

    Exocrine pancreatic insufficiency is treated with dose-standardised prescription pancrelipase, adjusted to fat intake and monitored. Over-the-counter blends are not potency-equivalent and using them instead can lead to malnutrition and fat-soluble vitamin deficiency.

  • Worth knowing

    Persistent symptoms need diagnosis, not enzymes

    Bloating, diarrhoea and abdominal pain have causes ranging from coeliac disease to malignancy. Taking enzymes to manage ongoing symptoms without investigation risks delaying a diagnosis.

  • Allergy

    Fungal and porcine sources

    Most supplement enzymes come from Aspergillus fermentation, which matters to people sensitised to mould. Prescription pancrelipase is porcine, which matters for religious and dietary reasons.

  • Too much

    High-dose enzyme effects

    Very high doses of pancreatic enzyme preparations have been associated with fibrosing colonopathy in children with cystic fibrosis, which is why prescription dosing is capped by lipase units per kilogram.
