Concentrated immunoglobulin G purified from bovine serum or from colostrum, sold as a powder. In the US, one serum-derived preparation is regulated as a medical food rather than as a supplement.

This is a category with an unusually clear split between two products that look identical on a shelf. Serum-derived bovine immunoglobulin is a defined preparation used under medical supervision for the dietary management of chronic diarrhoea and enteropathy, including HIV-associated enteropathy and diarrhoea-predominant irritable bowel, with small trials supporting improvement in stool frequency. Retail IgG powders sold as supplements are usually colostrum-derived concentrates of variable immunoglobulin content, tested only in small studies of gut permeability and exercise-induced barrier changes. The mechanism is plausible and local — antibodies binding microbial antigens in the gut lumen without needing to be absorbed — which is one of the more coherent stories in this catalogue, and still not the same as a demonstrated benefit for a healthy person.

The trials are small, several are open-label, and the results belong to the medical-food preparation rather than to retail IgG powder.

What may be going on: Immunoglobulins bind microbial antigens and toxins within the gut lumen without needing to be absorbed, which is a rare instance of a supplement mechanism that does not depend on crossing the gut wall.
