Bovine kidney concentrates cadmium, a metal that accumulates in the human kidney over decades with a biological half-life measured in years, and which damages kidney function and bone at chronic exposure. Several food safety authorities advise limiting offal consumption for this reason. A daily capsule of kidney powder is a deliberate long-term exposure and almost no brand publishes a cadmium assay.

A daily multi-organ dose supplies substantial heme iron and copper. Anyone with haemochromatosis, Wilson's disease or a high ferritin should avoid it, and most men and postmenopausal women have no reason to add iron at all.
