About Acetyl-L-carnitine

The acetylated form of carnitine, which crosses the blood-brain barrier more readily than carnitine itself. Sold for cognition and nerve health rather than for fat metabolism.

Acetyl-L-carnitine has a genuinely different profile from plain carnitine, because the acetyl group allows central nervous system entry and provides acetyl units for acetylcholine synthesis. The clinical literature is old and mixed: a meta-analysis of trials in mild cognitive impairment and early Alzheimer disease reported small benefits, though most of those trials were conducted in the 1980s and 1990s to standards that would not pass today. The strongest signal is in painful diabetic neuropathy, where trials at 2–3 g/day report reduced pain and some improvement in nerve conduction.

Meta-analysis of older trials in mild cognitive impairment and early Alzheimer disease reports small benefits on clinical and psychometric measures. The trials are old, small and heterogeneous, and no recent large trial has confirmed the finding.

What may be going on: The acetyl group is a substrate for acetylcholine synthesis and acetyl-L-carnitine supports mitochondrial fatty acid transport in neurons, though neither has been shown to mediate the clinical effect.

    Trimethylamine N-oxide production

    As a carnitine compound it is subject to the same gut bacterial conversion to trimethylamine and then to trimethylamine N-oxide, a metabolite associated with cardiovascular risk in cohort studies.

  • Interaction

    Warfarin and thyroid hormone

    Interacts withWarfarin, levothyroxine

    Case reports describe raised INR with carnitine compounds and warfarin. Acetyl-L-carnitine has also been reported to interfere with thyroid hormone action at the receptor level, which matters for anyone on thyroid replacement.

    Seizure threshold and agitation

    Carnitine compounds have been associated with increased seizure activity in susceptible people, and agitation or restlessness is a common complaint at higher doses.
