Meta-analysis of older trials in mild cognitive impairment and early Alzheimer disease reports small benefits on clinical and psychometric measures. The trials are old, small and heterogeneous, and no recent large trial has confirmed the finding.
What may be going on: The acetyl group is a substrate for acetylcholine synthesis and acetyl-L-carnitine supports mitochondrial fatty acid transport in neurons, though neither has been shown to mediate the clinical effect.
Limited evidence. Early human work, or mostly lab and animal research.
