About nopales

The young flattened stem segments of the prickly pear cactus, de-spined and cooked as a vegetable across Mexico. They are tart and slightly mucilaginous, somewhere between green bean and okra.

Trials in people with type 2 diabetes report meaningfully lower post-meal glucose when nopal is eaten with a meal, and this is one of the better-studied single vegetables for that outcome. Most trials are small, short and conducted in Mexico, and effects on longer-term markers such as HbA1c are much less consistent.

What may be going on: The viscous soluble fibre slows gastric emptying and glucose absorption; effects on incretin response have also been reported.

Moderate evidence. Several human studies point the same way, with real caveats.

    Additive effect with glucose-lowering medication

    Nopal lowers post-meal glucose reliably enough that combining large amounts with insulin or a sulfonylurea could push blood sugar too low. This is a reason to monitor and to tell your prescriber, not a reason to avoid the vegetable.

  • Too much

    Diarrhoea, bloating and nausea

    Trials of prickly pear preparations consistently report gastrointestinal side effects at higher doses — loose stools, increased stool volume, bloating and nausea. The mucilage that produces the metabolic effect is the same thing producing these.

  • Worth knowing

    Glochids

    Beyond the obvious spines, cactus pads carry glochids, tiny barbed hairs that lodge painfully in skin and are difficult to remove. Pads must be properly cleaned before cooking.
