About apple cider vinegar

Apple juice fermented twice — yeast turns the sugar into alcohol, then acetobacter turns the alcohol into acetic acid. Unfiltered versions contain the cloudy bacterial mass sold as the "mother". A tablespoon is the usual reference amount.

Apple cider vinegar is the most oversold item in this file, and the gap between what has been measured and what is claimed is wide. The measured part is narrow and real: acetic acid taken with a starchy meal blunts the glucose rise afterwards. Almost everything else attached to it — fat loss, skin, digestion, energy — rests on tiny trials, animal work or nothing at all, and the acid itself is capable of doing damage to teeth and the oesophagus if used carelessly.

Meta-analyses of small randomised trials report lower post-meal glucose and modest reductions in fasting glucose and HbA1c when vinegar is taken with meals. The trials are small, short and often poorly blinded — vinegar is hard to disguise — and the amounts used are one to two tablespoons daily, so the effect is real but small and easily overstated.

What may be going on: Acetic acid appears to slow gastric emptying and to inhibit disaccharidase activity, spreading carbohydrate absorption over a longer period.

Moderate evidence. Several human studies point the same way, with real caveats.

    Erodes tooth enamel

    Vinegar is acidic enough to dissolve dental enamel, and the damage is permanent. Sipping it neat or drinking it daily undiluted is the risky pattern; diluting well, drinking through a straw, rinsing with water afterwards and not brushing immediately all reduce it.

    Oesophageal and gastric injury from undiluted vinegar and tablets

    Case reports describe chemical burns to the throat and oesophagus from swallowing vinegar neat, and from cider vinegar tablets lodging on the way down. Anything containing acetic acid should be well diluted and taken with water.

  • Interaction

    Delayed gastric emptying, insulin and diuretics

    Vinegar slows stomach emptying, which is unhelpful in gastroparesis and can make insulin timing unpredictable in type 1 diabetes. A case of low blood potassium and bone loss has been reported with very large chronic intake, which matters alongside diuretics or digoxin.
